Chiropractic Care and Low Back Pain: A Look Back at the Manga Report
The Manga Report funded by the government of Ontario represented an important moment in chiropractic history. Read on to find out what it was and what they found.
According the World Health Organization, low back pain is the leading cause of disability globally. (1) Even back in 1993, the need for effective and cost-effective strategies to reduce this burden were desperately needed to address serious fiscal concerns in healthcare, a problem that continues today.
Toward this effort, in 1993, the Ontario Ministry of Health commissioned Pran Manga & Associates. (2) They were tasked with independently researching and assessing the effectiveness and cost-effectiveness of chiropractic management of low back pain. This group of highly respected health economists at the University of Ottawa prepared the largest and most thorough analysis ever at the time of the scientific literature on low back pain. The group had no conflicts of interest nor prior knowledge or experience with chiropractic.
To cut to the chase, the key findings of their investigation were that chiropractic management of low back pain is more effective, more cost effective, and produces higher levels of patient satisfaction than other approaches. Their key recommendations were that it made economic and therapeutic sense for the government of Ontario to fully fund chiropractic services under OHIP to remove the burden of the public to pay out of pocket. They also recommended full integration into the health care system, including hospitals. Importantly, the report specifically called out that the necessary changes would need to be instigated and monitored by the government, as the necessary changes would not spontaneously occur.
If such benefits of their findings and recommendations were so clear, it makes one wonder, why then did the government not take action to implement these high impact systemic improvements? Regardless, the findings of the Manga Report are a timeless exposé of how powerful the simplicity of the chiropractic approach can be in changing the lives of individuals, communities, and society at large.
